How do you get a copy of a previously filed PA Income Tax Return? To receive copies of previously filed tax returns, you should complete and sign Form REV-467 and send it to:
PA Department of Revenue If you are requesting the release of your own tax records, you may also fax your request to 717-783-4355. If another party is making the request on your behalf, they must submit to the department a copy of your authorization to request the release of your tax records. This usually will be a power of attorney on tax matters. If the taxpayer is deceased, sufficient evidence to establish that you are authorized to act for the taxpayer's estate must be provided to the department. The department will notify you if there are any fees associated with your request. Form REV-467 is available online at: http://www.revenue.pa.gov/FormsandPublications/otherforms/Documents/rev-467.pdf You may also submit a written request including your name, address, Social Security number, tax year, address used when filing your return (if different from your current address), and your legible signature authorizing the request. If you are requesting the return to prove you filed in PA that year, include that in your letter. Please allow four to six weeks for your request to be processed.
